Please follow these steps to complete and submit your forms:
- Download the required form(s)
- Open the form using Adobe Acrobat Reader. For full compatibility, do not use your web browser or other PDF viewers. Instead, open the form using Adobe Acrobat Reader, which you can download for free at get.adobe.com/reader.
- Complete all the required fields on the form(s) in Adobe Reader
- Submit the completed form by clicking on the ‘Submit by Email’ button on the top right-hand corner of the form(s)
